Health Histories

When you become a care leaver, you will be given a health passport by the looked after children nurse at your last statutory health check, which will contain all your health information.  Your health history will be a summary of your overall health and will be provided just before your 18th birthday.

This will be useful when you access healthcare services and adult services. Therefore it’s important that you keep this information safe as you may need it at some point in the future. 

If you have a learning disability, you are entitled to have a free Annual Health Check with your GP from the age of 14.
