You will have the support of your Personal Adviser when you get your permanent home who will help you settle in.

The Care Leavers 18+ Service will undertake a Pathway Plan Review when you have been in your new home for 28 days and at least once every 6 months after that.

Your Personal Adviser will visit within a week of you moving into your new home, and then at least every 2 months after that. They will assist and guide you with housing issues.
